Who Shouldn’t Use a TENS Machine? The Contraindications Worth Checking First

A TENS machine looks deceptively simple: a small battery-powered box, two or four sticky pads, and a dial. That simplicity is exactly why so many people buy one on a whim from a pharmacy shelf without reading past the first page of the instructions. Most adults can use TENS safely for everyday aches, but a genuine minority shouldn’t use one at all, and a wider group needs to place the pads with real care. This is the checklist a cautious buyer should work through before the first session, not after.

Who should not use a TENS machine at all

The clearest contraindications are also the most serious. Anyone fitted with a pacemaker or any other electrical implant near the heart should not use TENS, because the electrical current the device produces can interfere with how the implant senses and paces the heart. People with epilepsy are advised against it too, since the stimulation has the potential to affect seizure activity, and any use in a child with epilepsy needs specific clinical caution rather than a guess at the kitchen table.

The other absolute is pain that hasn’t been diagnosed. TENS is a symptomatic treatment: it can dull the sensation of pain, but it does nothing to treat whatever is causing it. Using it to mask an ache before a GP has worked out what’s actually wrong risks delaying a diagnosis that matters, particularly if the pain is new, severe, or doesn’t fit a pattern you already understand.

Where the electrode pads must never go

Even for people who are otherwise fine to use TENS, placement is not “anywhere it hurts.” A handful of areas are off-limits regardless of where the pain is:

  • The front or sides of the neck, and specifically over the carotid sinus
  • Over the eyes, inside the mouth, or on any internal surface
  • Directly over the spinal cord or near the heart
  • On the chest and the back at the same time, which lets current cross the chest cavity
  • Over broken skin, blisters, unhealed scars, or numb, infected or irritated patches
  • Over metal implants or jewellery, and never in the bath, shower, or anywhere the unit could get wet

These aren’t small-print technicalities. They reflect the fact that TENS works by passing a real electrical current through tissue, and the body’s more sensitive electrical systems (the heart, the nervous system) need a wide berth.

Situations that call for a conversation with a clinician first

Pregnancy is the case discussed most often, and the honest answer is that TENS should not be used in pregnancy unless a doctor or midwife has specifically advised it — the safety of stimulation earlier in pregnancy, and its effect before labour has actually started, has not been firmly established. This is a different situation from using a TENS machine during labour itself, which is a common and generally accepted use once a midwife is involved.

People with any kind of heart condition, not just a pacemaker, should check with a clinician before starting TENS, since the device can interfere with other electronic monitoring equipment such as ECG leads. Anyone whose skin reacts easily should also expect some risk: irritation at the pad site is the single most commonly reported side effect, and the standard advice is to move the electrodes slightly each session and to stop immediately if redness, itching or soreness appears.

Is a TENS machine actually a regulated product?

Yes — and this matters more than the marketing on a box suggests. In the UK, TENS machines are classed as medical devices and are required to carry a UKCA mark (or, under current transitional rules, a CE mark) before they can legally be sold. That marking is meant to show the manufacturer has satisfied requirements for quality, safety and performance, and the product should be registered with the UK’s device regulator before it reaches the market. A device with no visible conformity mark, no manufacturer’s leaflet, and no traceable UK contact for the seller is a product to walk away from, however cheap it looks.

None of this means TENS is dangerous for the vast majority of people who reach for one. It has no curative value and won’t fix the underlying problem, but as a way of taking the edge off everyday muscular or nerve pain, it has a long track record of safe use when the basic rules are followed. The point of a contraindications list isn’t to frighten people away from a genuinely useful piece of equipment — it’s to make sure the small number of people who shouldn’t use one, and the wider group who need to be careful about where the pads go, actually get that information before they open the box rather than after something goes wrong.

A short pre-purchase safety checklist

  1. Confirm you don’t have a pacemaker, another cardiac implant, or epilepsy
  2. Check the pain you want to treat has already been diagnosed, not guessed at
  3. Read the placement diagram before your first session, not during it
  4. If you’re pregnant, ask a midwife or GP before you start, not after
  5. Look for a UKCA or CE mark and a proper instruction leaflet before you buy

A TENS machine bought with these checks done first is a genuinely low-risk piece of home equipment. Bought without them, it’s a gamble with a product that, however small it looks, is still passing an electrical current through your body.
