TENS units & drug-free pain support

A small electrical pulse, a tingling sensation, and — for some people — a useful drug-free top-up to pain management. We explain how TENS actually works, what the settings do, who should steer clear, and what the evidence honestly shows. No hype, no hard sell.

What is a TENS machine?

TENS stands for transcutaneous electrical nerve stimulation. It's a small, battery-powered device that sends mild electrical impulses through sticky pads (electrodes) placed on your skin. You feel these as a tingling or buzzing sensation rather than as pain.

The idea is that those impulses can interrupt or reduce the pain signals travelling to your spinal cord and brain, and may also encourage your body to release endorphins — its own natural painkillers. TENS doesn't treat the underlying cause of pain; at best it offers temporary, drug-free relief that some people find a helpful addition to other measures.

It's widely used at home for backache, joint and muscle pain, period pain, and as a comfort measure in early labour. It is not a replacement for advice from your GP, pharmacist or a physiotherapist — see them about persistent or worsening pain.

How TENS works — and what the settings do

Most home units share the same core controls. Understanding them helps you use the device safely and get the most out of it.

Intensity (amplitude)

How strong the sensation feels. Turn it up slowly until you feel a firm but comfortable tingle — never to the point of pain. You may need to nudge it up over time as your nerves adapt.

Pulse rate (frequency, Hz)

High-frequency settings (around 80–120 Hz) are the most common for general pain relief. Lower frequencies (2–10 Hz) feel more like a tapping and are sometimes used to encourage endorphin release.

Pulse width (duration)

Controls how long each impulse lasts. Some machines fix this; others let you adjust it to fine-tune comfort.

Programmes & modes

Many units offer preset programmes (constant, burst, modulated). Burst and modulation modes vary the signal so your body is less likely to 'tune it out' during a long session.

Electrode placement

Pads go on clean, intact skin around — not directly on — the painful area, following the device instructions. Never place pads over the front of the neck, the eyes, broken skin, or the chest/heart area.

Who should NOT use TENS

Do not use a TENS machine — or speak to your GP or midwife first — if any of the following apply to you:

  • You have a pacemaker or any other implanted electrical or metal device.
  • You have a heart problem or heart-rhythm condition.
  • You have epilepsy.
  • You're pregnant — TENS is safe to use from 37 weeks onwards, but should not be used early in pregnancy, and never on the trunk or pelvis in the first trimester, unless a doctor or midwife advises it.

Also avoid placing pads over broken, numb or infected skin, over the front of the neck, over the eyes, or across the chest. If you're unsure, ask a pharmacist or your GP before you start.

What to look for when choosing a TENS unit

You don't need the most expensive device. Focus on safe, usable features rather than long spec sheets or marketing claims.

  • Adjustable intensity and frequency

    Independent control of strength and pulse rate lets you tailor the sensation. Avoid units with a single fixed setting if you want flexibility.

  • Clear, simple controls

    A readable display and obvious buttons matter — especially if you have reduced dexterity or you'll be using it during a flare-up or in labour.

  • Dual channels (if treating two areas)

    Two output channels let you place two pairs of pads — useful for, say, both sides of the lower back.

  • A safety/lock feature

    A keypad lock or guard prevents the intensity being knocked up accidentally, particularly handy for the labour 'boost' button.

  • Quality of the electrode pads

    Replaceable, reusable self-adhesive pads keep running costs down. Check pads are CE/UKCA marked and that replacements are easy to buy.

  • Battery type and life

    Rechargeable units save on disposables; battery-powered ones are reliable for travel. Either way, check it lasts a full session.

  • UKCA or CE marking

    A genuine medical-device mark shows the unit meets UK/EU safety standards. Treat unmarked, ultra-cheap imports with caution.

TENS is a low-risk, drug-free option that some people find genuinely helpful as part of a wider pain-management plan — but the evidence is mixed, and it won't suit everyone. NICE does not recommend TENS for chronic primary pain, and recent guidance advises against offering it routinely to women in established labour because trials haven't shown a clear benefit there. For early-labour backache at home, for short-term aches, and as a try-before-you-rely-on-it tool, an affordable, well-marked unit is a sensible buy. Just don't expect it to fix the root cause.

Pacemakers, heart conditions, epilepsy and early pregnancy are firm reasons to check with a professional first. Be sceptical of devices promising to 'cure' pain or build muscle.

TENS units: your questions answered

Does TENS actually work for chronic pain?

The honest answer is: it's uncertain. Large reviews of the research come to mixed conclusions, and NICE does not recommend TENS for chronic primary pain. That said, it's low-risk, and some individuals report meaningful short-term relief. It's reasonable to try as part of a broader plan agreed with your GP or physiotherapist — but not as a sole or long-term treatment.

Can I use a TENS machine during labour?

Many women use TENS at home in early labour, particularly for backache, and the NHS supports this. However, current NICE guidance advises that TENS should not be offered to women in established (active) labour, as trials haven't shown a clear benefit at that stage, so it isn't routinely provided in hospitals. TENS is considered safe from 37 weeks of pregnancy — discuss it with your midwife.

Is TENS safe?

For most healthy adults, used correctly on intact skin, TENS is considered low-risk. The key exceptions are people with a pacemaker, other implanted electrical/metal devices, a heart condition or epilepsy, and those in early pregnancy — all should seek professional advice first. Never place pads over the front of the neck, the chest, the eyes or broken skin.

Will it interfere with painkillers or other treatment?

TENS is drug-free and can usually be used alongside medication, but it isn't a substitute for treating the cause of your pain. If pain is new, severe, worsening, or unexplained, see your GP rather than relying on a device.

How much should I expect to spend?

Basic home units are inexpensive, and the ongoing cost is mainly replacement pads. Spending more buys extra programmes, dual channels and rechargeable batteries — not necessarily better pain relief. Choose for usability and a genuine UKCA/CE mark, not price.
